Limited Supply of H1N1 Flu Shots Arrive at DVUSD Schools
The Deer Valley Unified School District (DVUSD) is partnering with Mollen Immunization Clinics to provide the H1N1 flu vaccine to students in the district. Due to the limited vaccine supply arriving slowly in Maricopa County from the federal government, DVUSD is receiving limited amounts from Mollen on a weekly basis. Mollen has provided the district with the first batch of immunizations authorizing DVUSD to offer it to enrolled PreK-6 students only at this time. DVUSD will be offering the H1N1 flu vaccination during school to students ages four and older enrolled at Constitution, Desert Winds, Sunrise, and Village Meadows Elementary Schools on Friday, November 6th from 9 a.m. � 2 p.m. No other DVUSD Schools are scheduled at this time.
Parents must register their child online with Mollen at http://consumer.flushotsusa.com/schools.aspx and follow the registration steps. Parents must print out and sign the parent consent form and return to their school office. The cost of the immunization is $15 and can paid by credit card or be billed to the Arizona Health Care Cost Containment System (AHCCCS) or an individual�s health insurance company. Parents can refer to www.flushotsusa.com to verify if their insurance is accepted.
If parents do not have access to a computer, they can visit their school office or district office located at 20402 N. 15th Ave., Phoenix 85027 for assistance. If parents are unable to participate in this program, the district is encouraging them to talk to their family doctor or visit health clinics about receiving the H1N1 immunization.
